We know that health care discussions with friends, family members and medical teams can be difficult, often confusing and frustrating.

  • How can we all do better?
  • What role does the patient or family member play?
  • What questions need to be asked?
  • What elements are necessary for a thoughtful, sensitive conversations, especially around patient wishes?

We want to learn how conversations with you can be more user friendly. During our time together, information and dis-cussion about Advance Care Planning documents and Physi-cian Orders of Life Sustaining Treatment (POLST) forms will be included.
